Welcome to World of Wonders!

We're so excited to welcome your family to our community! This page will guide you through everything you'll need to complete before your child's first day.

Please complete each step below. Unless you've arranged an immediate start, all items should be completed at least seven days before your child's first day.

Step 1: Complete these forms for each child in your family

Step 2: Complete these forms once for the whole family

Family Needs Survey

Payment Agreement

Authorized pick up form

PFCC Payment Agreement form

Only fill out this form if your family receives PFCC/Title XX/Childcare Choice funding

Step 3: Send required documents

Email all of these to info@wowchildcare.org

A recent picture of your child
A family picture
Your child's most recent immunization record

Immunization records must include:

Your child's birthdate
A list of immunizations received
The date each immunization was given

If you cannot obtain the immunization record before your child's first day, that's okay. Please send it to us within 30 days of enrollment.

Step 4: Read Parent Handbook

This was given to you during your registration visit!

Step 5: Watch our parent orientation (optional)
